CORI

State law and the Diocese of Springfield mandates that CORI checks be implemented for all employees and volunteers who provide services to children, the elderly or disabled persons. The Faith Formation program requires all employees, volunteer teachers or any other volunteers to complete the CORI requirements.

The requirements are:

  • Submit a completed CORI Request Form with a copy of a government issued ID (i.e., driver’s license) to the Parish Office (Note:  The last 6 digits of a Social Security number are required to process a CORI form.);
  • Read the Diocesan Code of Conduct, signing the last page of this document and returning to the Parish Office;
  • View the “Protecting Our Children” film (available in the Parish Office). Please provide the date of the viewing on proper form and return it to the Parish Office;
  • Read the Child Abuse Reporting Fact Sheet provided by the Diocese.
